Immunizations Requirements by Grade Level
See below for requirements according to grade level:
(Students enrolling under provisions of McKinney Vento Homeless Assistance Act have 30 days to provide proof of immunization after submitting enrollment)

| Varicella (Chickenpox) If the student has had Varicella, a health care provider may sign off on the immunization. |
